Our Methodology: How We Selected the Best Medical Alert Watches
We decided the best way to compare and rank medical alert smartwatches is to test them ourselves. That way, we can fully grasp how each option would work in different scenarios and see their features in action.
We tested a total of eight smartwatches with medical alerting functions within a span of 12 weeks. We wore them all day, tested every function and feature, and ran medical emergency simulations to see how they would perform. Our team used the following criteria to identify the best three watches:
Value: To assess the value of each watch, we bought them ourselves and then compared the cost to its available features and hardware quality.
Medical alert functionality: We also assessed how effective they would be at saving lives by testing how easy it is to activate a medical alert, whether or not automatic fall detection is available, and other features related to medical emergency alerting.
Ease of use: Living with and actually using those medical alert smartwatches has revealed to us which of them are easiest to use, even for seniors who might struggle with advanced technology.

The MGMove is a round-faced smartwatch from Medical Guardian that features a large SOS button on the side, a 1.25-inch touch screen watch face, and built-in 4G connectivity. It’s also rated IP67, which means you can wear it in the shower.1
MGMove’s medical alert features caught our attention. Its built-in 4G connection meant that we didn’t have to carry a smartphone around to use it – perfect for seniors who prefer older style phones. The built-in GPS, on the other hand, provides emergency responders your real-time location. We ran tests outside the house several times, and the location data was always accurate; only off by 12 feet at most. Those features, alongside Medical Guardian’s fast response time, makes it one of the top mobile medical alert systems.

We bought our MGMove up-front for $199.95, which is pricey. The LifeStation Sidekick Smart cost us only $99.95. On top of the equipment fee, we’re paying $42.95 per month to keep MGMove connected to Medical Guardian’s TMA Five Diamond Certified monitoring center.2 The good news is you can pay three or 12 months at a time to lower the monthly cost. The price goes down to $38.95 per month with yearly billing.

We found MGMove worth it. Buying the device gave us access to a caregiver app, which the Sidekick Smart lacks. We also think it has a better build quality – lightweight but weather-resistant. And overall, it has better features than most medical alert watches. Fall detection is available for an extra $10 per month, and for an additional $5, you’ll get access to the Messages and Reminders apps.

Using MGMove During Medical Emergencies
The emergency button on the MGMove is set up in a way that prevents accidental triggers. It’s large and easy to press, but you’d need to hold it for five seconds to trigger a response. As we noted in our MGMove review, it takes about 23 seconds from pressing the button to getting connected to Medical Guardian.

We like the built-in speaker and mic for two-way talk with monitoring agents. At home, we could always hear the line clearly. However, it’s not loud enough in crowded outdoor settings. Fortunately, we know from testing three Medical Guardian products that the monitoring center will still get you help even if communication breaks down. And with MGMove’s built-in GPS, they can locate the wearer easily.
Speaking of locating the wearer, caregivers can also monitor the watch’s whereabouts using the MyMedicalGuardian app. However, location data is only updated hourly and not in real-time, likely to save battery. If you’re a caregiver and tracking real-time location is important to you, consider an Apple Watch instead. It’s our third pick for this list.
Something to Consider: As an add-on, Medical Guardian offers the Elite 911 – a small speaker box you can use to speak with monitoring agents during emergencies. It has larger speakers than the smartwatch, so it could work better in noisy settings. You can trigger the alarm from your smartwatch and then use the Elite 911 for two-way talk.

“In my experience as a nurse practitioner working with people with memory loss (and their families), there have been several instances where GPS tracking has prevented harm or been life-saving. All of these instances involve people who wander. One evening, one person living in an urban setting had made their way to an on-ramp to a local highway and was walking near the guardrail when a concerned family member “pinged” the location, determined where the person was, called the local police to give them the exact location, and within 15 minutes the wandering person was safely brought back home.”
We’ll get to the point – the SOS Smartwatch has the exact same hardware as MGMove. This means you get most of the same features if you go with Bay Alarm’s smartwatch, save for a few missing options. For example, you can’t load the Messages and Reminders apps onto it, even if you’re willing to pay an extra monthly fee.
What sets the SOS Smartwatch apart is the price. Although it retails for the same price as the MGMove, Bay Alarm packages are discounted more often. The monthly fee is also slightly lower, making the SOS Smartwatch a more budget-friendly option.

Having spent the last half decade testing Bay Alarm products, we know it offers deals more often than Medical Guardian. In fact, we got our SOS Smartwatch for only $159 – 20 percent less than the retail price. And for the monthly fee, the base cost without any add-ons is $39.95. Just like MGMove, adding fall detection costs extra $10 per month.
We also appreciate the bundle options. For $64.95 a month and $199 upfront (cost of the smartwatch), you’ll get the SOS Smartwatch for on-the-go protection and a cellular-based in-home system. That’s a sweet deal.
Expert Tip: The SOS Smartwatch’s 24-hour battery life is limited, so using a dedicated in-home system while you’re at home makes sense. The in-home system that comes with the bundle even comes with a wristband holder for the SOS button so you can wear it like a watch.
Using the SOS Smartwatch During Medical Emergencies
Since it has the same hardware as MGMove, our SOS Smartwatch experience was similar. It was easy to call for help, and two-way audio is available on the watch itself. GPS location tracking was also as accurate as Medical Guardian’s smartwatch.

That said, Bay Alarm offers limited add-on options for the SOS Smartwatch. You can get a lock box for $30, a Vial of Life kit for $8, and an extra charging dock for $49. Medical Guardian offers more for the MGMove, including additional smartwatch apps and the Elite 911 speaker box mentioned above.
On the plus side, we found Bay Alarm’s response time to be faster than Medical Guardian at around 21 seconds (including the 5-second button hold to trigger the alarm) compared to 24 seconds. Plus, when we bought our SOS Smartwatch, we got to choose whether to use the AT&T or Verizon network.
Fall Detection Test: We compared MGMove and SOS Smartwatch’s fall detection accuracy and got similarly convincing results. MGMove detected nine and SOS Smartwatch detected eight of the 10 simulated falls. See more Bay Alarm vs. Medical Guardian comparisons here.
The Apple Watch isn’t exactly a medical alert smartwatch, but you could use it to summon help using the Emergency SOS feature. All you need is an iPhone on which the watch can piggyback for cellular connectivity – or if you have a cellular-version, an eSIM set up for your Apple Watch.
We made it our third pick because of its impressive fitness tracking. According to the Center for Disease Control, physical activity has significant benefits for older adults, including better sleep, lower risk of developing heart disease, and improved independent living.4 Medical alert smartwatches don’t offer fitness tracking as comprehensive as the Apple Watch, so if your goal is to stay fit while having access to help when needed, it’s what we recommend.

It’s no secret that the Apple Watch is expensive, even compared to other non-medical alert smartwatches. The cheapest model, the Apple Watch SE, starts at $249, but we recommend the GPS + Cellular version that costs $299. We tested the Apple Watch 10, which retails for $499 for the GPS + Cellular model. That’s more than double the price of the MGMove and SOS Smartwatch (which both have GPS and cellular built-in).
On the plus side, you don’t have to pay a monitoring fee, so you could break even in the long run. One year of Bay Alarm monitoring for the SOS Smartwatch is already $479. The only monthly fee you have to pay is the cellular plan, which usually costs about $10 per month depending on the carrier.
Money-Saving Tip: If you’re already an iPhone user, you don’t need the GPS + Cellular model. The cheaper GPS model will do. Connect it to your iPhone and you’d be able to use the Emergency SOS function.
Using The Apple Watch During Emergencies
Our biggest doubt is the Apple Watch’s performance in medical emergencies. Emergency SOS essentially calls 911 from your Apple Watch. This means the response will be speedy. However, since 911 isn’t a dedicated medical emergency response center like Medical Guardian and Bay Alarm’s monitoring centers are, the quality of the response can vary.
On the positive side, you can activate fall detection for free. You don’t have to pay extra like you do with Medical Guardian. Apple claims the watch can detect hard falls and call emergency services on your behalf if it detects immobility for about one minute following the fall. In testing, though, we found it inaccurate. It detected only six of our 10 simulated falls.
Overall, we wouldn’t recommend using the Apple Watch as a medical emergency watch to older adults who are prone to emergencies. It’s not a medical alert smartwatch. We recommend it to those who are fit and active and want to stay that way, while gaining easy access to 911 just in case something happens.

As you’ve seen, medical alert smartwatches can cost $200 or more. Meanwhile, mobile medical alert systems cost $150 or less. Some don’t even charge equipment fees, like Life Alert’s Home & GPS plan.
2. Speaker Quality
Traditional mobile medical alert systems are often the size of a small walkie-talkie, which means they have bigger speakers than most smartwatches do. Usually, that translates to better sound quality especially when in crowded or noisy areas. That’s one of the downsides we noticed when we tested the MGMove and SOS Smartwatch.

Medical alert smartwatches have clear advantages over mobile medical alert systems, too. One is style and mobility. Many mobile medical alert systems require you to carry around a mobile base unit separate from the SOS button. It can be a hassle. Meanwhile, medical alert smartwatches are all-in-one. You simply wear them around your wrist.
